You need an application in the prescribed format, income proof for the relevant financial year (salary slips or ITR), and a declaration of family assets.
Address and identity proof are also required.
The certificate must be issued by the competent authority (Tehsildar or SDM) in the central government format.
It must be valid for the financial year mentioned in the UPSC notification — apply well in advance.
